Complex Case Management

The Complex Case Management Program offers more concentrated care for those who suffer from multiple chronic conditions and catastrophic illnesses. These individuals can face a variety of complications and may require coordinated care, pain and symptom management, advance or terminal care, assistance with daily living, or a range of other types of support.

Alere seeks to meet the greater needs of these participants by connecting them to the most effective tools and services available.

In addition to delivering financial, clinical, and operational results, it also demonstrates positive changes in patients' health-related behavior.

Participant satisfaction consistently rates above 90 percent, and Alere savings are achieved via reductions in healthcare utilization after the care team has provided education, promoted self-management skills and established a strong support structure. In fact, the program was recognized in 2009 with The Care Continuum Alliance "Quality Impact Award for Greatest Impact on Health."
