Alere Medical helps patients with chronic illnesses manage their conditions through a unique combination of at-home monitoring, patient education, and nurse-patient relationships. 
 
Our vision is to participate in a healthcare system that is efficient and integrated, so patients who are suffering from chronic illnesses can function independently and lead rewarding and productive lives at home.

Our mission is to improve the lives and outcomes of people with chronic illnesses by connecting patients’ objective findings and subjective symptoms to caring health professionals, who use evidence-based programs to educate patients about their illness and treatment. This connecting and caring empowers patients to lead healthier and more satisfying lives.

Founded in 1996, our mission began with a focus on improving care management for the millions of Americans diagnosed with heart failure. Our Heart Failure Program is a cost-effective, at-home monitoring program utilizing our patented DayLink™ Monitor. It provides physicians with real-time and actionable information about changes in patients' conditions, to reduce the risk of crises such as emergency care or costly hospitalization. Patients on the program also benefit from outbound nurse support and educational materials. The result is reduced hospitalizations, cost savings, and positive clinical outcomes.

In 2005, the scope of our mission expanded dramatically when Alere entered into a long-term disease management contract with PacifiCare Health Systems, Inc., resulting in a significant expansion of Alere’s size and service offerings. The transaction expanded Alere's disease management services to include additional disease categories, moving beyond its proven approach for heart failure to provide disease management for high-prevalence, high-cost diseases. Included in this expansion was Alere’s development of its Heart Smart Solutions Program for coronary artery disease (CAD), designed to complement and build upon the proven approach of Alere’s Heart Failure program.

Alere further extended its expansion into multiple disease management categories in 2005 when it acquired the disease management programs of National Jewish Medical and Research Center, specializing in asthma and chronic obstructive pulmonary disease (COPD). As a recognized industry leader in respiratory treatment and research, National Jewish Medical and Research Center’s disease management programs further established Alere as a diversified and nationally recognized disease management company.

Alere’s disease management services expanded yet again in 2005 as Alere partnered with Joslin Diabetes Center to co-develop a diabetes disease management program. By partnering with Joslin Diabetes Center, a global leader in diabetes research affiliated with Harvard Medical School, Alere gains access to over 100 years of expertise in providing high-quality, cost-effective, outcome-focused care for people with diabetes.
