Alere Disease Management Programs


The Alere Heart Failure Program  improves quality of life for heart failure patients by incorporating Alere’s patented DayLink™ Monitor, daily monitoring of high-risk members by cardiac nurses, notifications to the member’s physician about clinical changes, and “teachable moment” interactions between nurses and members, empowering them to be active participants in managing their disease.

The Alere Heart Smart Solutions™ Program  identifies patients at risk for cardiovascular disease and provides approved guidance on lifestyle changes. This program focuses on the key cardiac risk factors: “Knowing Your Numbers,” medication adherence and compliance, and smoking cessation.

The Alere Diabetes Program powered by Joslin Diabetes Center  is developed in collaboration with Joslin Diabetes Center, an affiliate of Harvard Medical School and the largest institution dedicated to diabetes research, care, education and external programs. The program includes health risk assessments, risk stratification methodologies, provider engagement approaches, patient education and outreach, clinical guidelines, and programs connected to co-morbidities.

The Alere COPD Program powered by National Jewish  was developed in conjunction with National Jewish Medical and Research Center, the #1 Respiratory Hospital in the US (1). Clinical interventions are based on comprehensive assessment by respiratory nursing staff, in conjunction with the member’s physician(s).

The Alere Asthma Program powered by National Jewish  was developed in conjunction with National Jewish Medical and Research Center, the #1 Respiratory Hospital in the US.1 It utilizes a comprehensive clinical assessment by specialized respiratory nursing staff. Interventions are determined based on this assessment and in conjunction with the member’s physician(s). Clinical care managers educate members to encourage positive behavior change and provide comprehensive medication information.
