Alere Asthma Program

The Alere Asthma Program powered by National Jewish begins with a comprehensive assessment asking members a series of specific clinical and demographical questions to determine the appropriate level of support. Based on this assessment and in conjunction with the member’s treating physician(s), interventions are determined based on a member’s symptoms and preferences. Members may move between program levels as necessary.
All members are provided with an Asthma Action Plan, describing an asthmatic episode and defining early warning signs. Registered nurses educate members about methods to identify and avoid triggers, encourage positive behavior changes and provide comprehensive medication information.

Types of Interventions
- Assessment questionnaires for risk categorization
- Assessment for individual and second-hand tobacco exposure and on-going issues
- Initial telephonic comprehensive clinical assessment and followed by coaching calls (frequency determined by clinical status) for high risk patients
- Individualized Action Plans which describe self assessment and management strategies for all levels of severity and are coordinated with the PCPs recommendations
- Discussion related to environmental triggers, allergies and exercise- induced asthma
- Full educational program including tailored education calls
- Peak flow meter monitoring
- Series of asthma education booklets
- Physician / Case Manager summary reports
- Quality of Life Survey-SF12v2™ at enrollment and Q6 months thereafter
- Patient Satisfaction Survey Q6 months
