Alere® Heart Smart Solutions™ Program

For Coronary Artery Disease

This program focuses on the key cardiac risk factors: high blood pressure, high cholesterol, smoking, and glucose control for diabetics. By "Knowing Your Numbers" patients become aware of their cardiac risk score as assessed by the Framingham risk assessment tool. Then through improved medication adherence and compliance, smoking cessation, and lifestyle changes patients achieve a reduction in their cardiac risk score and improved health.
 
Through the use of PBM data which are compared to national guidelines, care gaps are identified and recommendations are made based on those guidelines. Educational materials are provided and members are encouraged to participate in tobacco cessation programs as appropriate.

Types of Intervention
  • Initial comprehensive telephonic assessment and reassessment every 1 or 4 months depending on intervention level, and more frequently as required by PBM data monitoring, by cardiac trained registered nurses
  • Pharmacy benefit manager (PBM) data monitoring monthly
  • Pre-visit physician reports
  • Registered Nurse telephonic coaching & education
  • Collection of key health numbers (LDL, BP, cholesterol)
  • Smoking cessation support
  • Patient reminders and follow up for annual vaccinations
  • Full educational program including tailored education calls
  • Series of CAD education booklets
  • Patient Incentive Program - Patient Recognition And Incentive Support (PRAISe™) Program
  • Quality of Life Survey-SF12v2™ at enrollment and Q6 months thereafter
  • Patient Satisfaction Survey every 6 months
