Alere has collaborated with Joslin Diabetes Center to offer a diabetes program that provides a full range of disease management content and expertise.
Joslin Diabetes Center is an affiliate of Harvard Medical School, and the largest institution dedicated to diabetes research, care, education and external programs. Alere deploys a range of services as a partner with Joslin including health risk assessments, risk stratification methodologies, provider engagement approaches, patient education and outreach, clinical guidelines, and programs connected to co-morbidities.

Types of Interventions
- Initial telephonic comprehensive clinical assessment and followed by coaching calls (frequency determined by clinical status)
- Pharmacy benefit manager (PBM) data monitoring monthly
- Patient data review by Diabetic-trained registered nurses
- Diabetic-trained nurse telephonic coaching & education
- Collection of key health numbers (HbA1c, BP, cholesterol)
- Blood sugar monitoring at home using Alere’s patented DayLink™ Monitor
- Sick day interventions as indicated
- Patient reminders and follow up for annual eye and foot exams
- Patient reminders and follow up for annual vaccinations
- Comprehensive dietary and weight control online tools
- Full educational program including tailored education calls
- Pre-visit physician reports & alert reports
- Quality of Life Survey-SF12v2™ at enrollment and Q6 months thereafter
- Patient Satisfaction Survey every 6 months
