Alere® Heart Failure Program
The Alere Heart Failure Program improves the quality of life for heart failure patients by incorporating monitoring (daily for high risk patients) of weight changes and other symptoms of heart failure focusing on adherence and compliance of their physician’s treatment plan.
“Teachable moment” interactions occur between nurses and members, empowering them to be active participants in managing their disease while preventing unnecessary hospitalizations.
For high risk patients, the DayLink™ Monitor is shipped directly to the member and is simple to set up. The member is asked to step on the scale and answer questions twice a day, and the monitor then transmits the information over the member’s phone line.
Alere’s disease management nurses review the data reported daily. If there is a weight or symptom change, the nurse telephones the member to verify the information and then notifies the designated health care providers by fax. The nurse contacts both the member and physician to ensure that the follow-up occurred in a timely manner.

Types of Interventions
- Telephonic assessment and reassessment as frequently as required by weight and symptoms needs
- Home monitoring via our DayLink® Monitor or telephonic interaction
- Assessing the patient’s weight and/or symptoms seven days a week, 365 days a year
- Cardiac-trained nurse interventions if the patient’s weight and/or symptoms exceeds parameters
- Physician alert reports if weight or symptoms are verified to be “out of parameters”
- Telephonic education as needed per print and web-based education via our Smart Living for Your Heart™ program
- Quarterly Newsletter
- Quality of Life Survey-SF12v2™ at enrollment and Q6 months thereafter
- Patient Satisfaction Survey every 6 months
