A key feature of the Alere program is personalized patient education that improves patients' understanding of heart failure and empowers them to better self manage their illness. Daily monitoring allows for real-time education at the point when patients need it most and reinforces the connection between behavior, symptoms and hospitalizations.
Physicians with patients on our program have found an increase in patient adherence to treatment plans; overall improved patient awareness of their illness; and a reduction in avoidable hospitalizations.
Patients surveyed after 3 months on the Alere® Home Monitoring Program have reported a significant reduction in impairment and improved quality of life.
